Understanding Proprioception


What is Proprioception???

Proprioception.....  ज्यादातर लोगों के लिए ये एक नया और अनसुना शब्द होगा। कुछ पेचीदा से लगने वाले इस शब्द को समझाने के लिए आज हम कुछ examples की मदद लेंगें। 




 सबसे पहले, वो driver जो आँखें सड़क पर रख कर भी बिलकुल सटीक तरीके से अपने हाथों को steering wheel पर और पैरों को brakes पर बनाये रख लेते हैं। 



दूसरा, वो musician जो आँखेँ बंद कर के भी अपने instrument को पूरी लय में बजा लेते हैं।


और तीसरा, सबसे common हम अँधेरे कमरे में भी खुद को किसी चीज़ से ठोकर खाने से बचा कर light के switch तक पहुँच ही जाते हैं।  


कभी सोचा है कि, बिना नज़रों का इस्तेमाल किए हम ये सब कैसे कर लेते हैं? कैसे हमारा दिमाग बिना किसी visual input के भी हाथों और पैरों को सही जगह पहुंचा देता है और सही काम पर लगा देता है? यदि हमें हमारे रोज़ भी काम के दौरान अपने हाथों और पैरों पर नज़र रखनी होती तो, उनकी स्तिथि और दिशा को हर पल monitor करना होता तो हमारे लिए रोज़मर्रा के काम करना कितना मुश्किल होता। ये खुदबखुद नहीं होता। ईश्वर ने हम humans को इसके लिए एक स्पेशल sense दिया है जिसे proprioception कहते हैं। 
Proprioception को body position sense कहते हैं। ये वो secret special sense है जो हमें हमारे body का सही orientation in space और body parts की correct position बताता है और उन्हें control करने में हमारी मदद करता है। 

अब प्रश्न है कि ये special sense काम कैसे करता है? 
Proprioception के लिए कोई special organ नहीं है बल्कि ये special sense पूरे  body के coordination में operate करता है। हमारी body की हर muscle में कुछ ख़ास receptors होते हैं (muscle spindle और golgi tendon organ) जो हमारी muscles का tension और force of contraction को monitor करता है और ये signals लगातार brain को भेजते हैं। Brain के एक special हिस्से, cerebellum में पहुँचने वाले ये signals की processing से brain हमारे limbs की सही position का अंदाज़ा लगाता है। इस तरह से proprioceptive system बिना visual और auditory input के भी limbs की correct position का हिसाब लगा लेता है। हालाँकि visual और auditory inputs की मौजूदगी से इस सिस्टम की accuracy और भी बढ़ जाती है।  


Proprioception किसी भी और sense की तरह training से improve किया जा सकता है और disease या इस्तेमाल ना किए जाने पर कमज़ोर हो सकता है। इसीलिए physical therapist proprioception को train करना एक important rehabilitation goal मानते हैं ।

How does midline crossing evolve?: the building block of midline crossing skills

Midline crossing skills develop with an interdependence of 5 primary skills and have direct relationship with each of them. The 5 building blocks of midline crossing skills are: 



Bilateral integration
Bilateral integration is the ability to use the right and left sides of the body together in a coordinated way. The movements of both sides of body together in activities is only possible by processing and integration of both hemispheres of the brain together at the same time. Therefore, good bilateral integration indicates that both sides of the brain are communicating effectively and sharing information. Bilateral integration is seen in gross, fine motor and functional skills. 

The Three Types of Bilateral Movements

Core stability 


Core is the musculature that surrounds the lumbopelvic region, have attachments with thoracolumbar fascia and spinal column and connect the upper and lower limbs.


The core musculature comprises of:
Anteriorly: abdominals
Posteriorly: paraspinals and gluteals
Inferiorly: pelvic floor muscles
Laterally: hip abductors and rotators
Superiorly: diaphragm.


     
     Core muscle group maintains upright and erect posture. It can be exemplified as the trunk of a tree. As the trunk holds the tree to the ground, the core acts as the base of support for your entire body so that your branches (arms and legs) can work efficiently. The core  surround and support the spine and pelvis and connect your upper body and lower body, effectively transferring forces from one to the other.

Planning and sequencing of movements 

Planning and sequencing involves planning and executing a muscle action. It is also called praxis. It is a thee step process:
For example, for a task of picking up a pencil; the first step is to generate the idea or intercept the instruction of picking up the pencil. The second step is to process the command and program the steps to pick up the pencil, this includes sequences of muscle contraction and joint movements. The final step is execution which includes relaying of motor signals to respective muscles to contract and perform the task of picking up the pencil.
   This processing requires integration of the brain and the senses (e.g. touch, movement, vision, hearing) 


Praxia or planning and sequencing is important for all gross and fine motor activities and functional tasks expected for daily and school based activities of children. It helps them to perform daily activities with precision and assists in learning new tasks.




Planning and sequencing is an acquired skill developing from components of neuromotor skills. the is a sum total of characteristic features of musculoskeletal and neurological systems.

Body awareness
Body awareness is the internal body “map” which gives internal understanding of where the body is in space. 
Integrated processing of repeated sensory inputs and movement components helps in creation of body map over a period of time. 
Therefore, the body map or skills of body awareness is dependent on sensory inputs, primarily proprioceptive inputs from the joints, muscles, and connective tissueBody awareness is highly influenced by proprioceptive processing, the sensory information one receives from the movement and force of muscles and joint groups. 
Proprioception is the internal sense that tells you where your body parts are without your having to look at them. Proprioception, the awareness of deep pressure and the position and movement of limbs, is mediated through receptors in muscles, tendons, and joints. Proprioception encompasses three aspects, known as the ‘ABC of proprioception’. These are: agility, balance and coordination.

Hand Dominance

Hand dominance describes a child’s natural inclination for favoring one hand over the other for skilled activities such as functional activities and academic tasks. For most children, hand preference begins to emerge between the ages of 2 to 4, and usually by the age of about 6, most children will develop a preferred hand. Developing a dominant hand is important to efficiently perform tasks involving midline crossing and bilateral integration skills, where one hand needs to act as a helper to the dominant hand.

Why does midline crossing evolve in the first place?: the significance of ATNR

Midline crossing is a complex functional approach directed towards reaching the opposite side of body. Midline crossing refers to any motor action that results in looking, reaching, or stepping across the body's midline. The evolution of this motor activity is aimed to integrate the left and right sides of body and brain, as explained in previous blog. Why does midline even exist?

Apart from being an important developmental milestone midline crossing is essentially laid down as a developmental skill succeeding the integration of a primitive reflex- ATNR. 

ATNR (Asymmetrical Tonic Neck Reflex) is a primitive reflex that emerges at 18th week of intra-uterine life. This reflex helps in intra-uterine development of muscle tone and vestibular system and helps the baby during birthing process by inhibiting limb movements while the baby turns in the birth canal during delivery. This reflex can be identified within the first 3 months of life. And after birth, it ensures head tilting to the side while the baby lies on their stomach to keep their airways clear, and is the first step toward developing hand-eye-coordination. 



The reflex is initiated when the head is turned to the left or the right whilst the baby lies on its back. The turning of the head causes the arm and the leg on one side to extend, whilst the limbs on the opposite side flex. For example, if the head turns to the right, the right arm and leg will automatically extend whilst the left arm and leg both flex.



The ATNR is integrated at the age of 6 months and replaced by complex functional movements by which the baby begins to reach within the mid line of their body.
 If due to any cause (which are extensive ranging from birth problems to delayed brain development) the reflex is retained it will continue to influence limb movement every time the head is turned and will have influences on  physical movements. A retained ATNR will affect the functional reaching abilities and the baby finds it difficult for hands, feet and eyes to cross the mid line. Retained ATNR interferes with a baby's ability to centralize and reach things within the midline of body and prevents the baby from developing crossing pattern movements such as rolling over, crawling and later marching using alternative leg and arm.
The most distinct sign of a retained ATNR is the inability to cross the midline.

Why does this midline even exist?.....the neurological approach


The body's midline is a concept of visualization and revolves around the structural and functional anatomy of brain. Our brain that we consider as a single organ is actually made up of two separate hemispheres: the left and the right hemispheres, separated by a structure called longitudinal fissure.

But the two hemispheres are not actually separable as they are connected by a structure called corpus callosum. It relays information from the left hemisphere to the right hemisphere and vice versa serving as a big information highway between the two hemispheres of brain.



So as an irony, we have two separate brain hemispheres which are actually not separate but connected.... why so? So the two hemispheres, or the left and right brain are actually responsible for proper functioning of the body. The left side of the brain is responsible for controlling the right side of the body. It also performs tasks that have to do with logic, such as in science and mathematics. On the other hand, the right hemisphere coordinates the left side of the body, and performs tasks that have do with creativity and the arts. Both hemispheres are connected by the corpus callosum and serve the body in different ways. 

Therefore, the entire body and brain actually have two interconnected halves. As in brain the corpus callosum serves the function of inter-connectivity and interdependence, on physical level, the midline crossing activities serve the function of connecting the two halves of body and stimulate integration of left brain with right body and vice versa. 

 Midline activities are important to help coordinate the two sides of the body together and encourage communication between the right and left sides of the brain. Bilateral coordination is an important step in developing midline crossing.
 Development of bilateral skills, both symmetrically and asymmetrically, helps in development of midline crossing begins. 
A child with midline crossing difficulty, finds it difficult to integrate the two halves of body and brain and as a result each side of the body tends to operate on its own. Children who avoid crossing the midline work the right side of the body with the right hand and the left side of the body with the left hand or they turn their body to avoid crossing the midline. This interferes with the normal development of a dominant hand and makes bilateral motor tasks very difficult.
